Facebook friend adding glitch?

when i add someone on facebook, sometimes (not all the time) i get posts on my newsfeed saying "so and so is now friends with ____ and 3 other people." but the problem is, "so and so" i didn't actually accept my friend request because every time i go on their page it says the friend request was already sent and pending. so basically, its like i'm getting a notification from someone that hadn't accepted me as a friend yet. is this happening to anyone else? please help!
